ESTABLISHING TELEPHONE SERVICE

 

Visit or call our Business Office – 811 toll free within Lincoln County – 800-340-4131 toll free outside Lincoln County.  Representatives in our Business Office can assist with orders for service and equipment, billing matters, rates, and inquires or complaints.  Our representatives are there to see that you are completely satisfied.

 

INFORMATION NEEDED FOR YOUR ORDER

 

• Completed Application (Front & Back)
• Check Desired Custom Call Features
• Check Desired Long Distance Carrier – Carrier Must Have an Access Service Request with LCTS.  Indicate Toll Carrier’s PIC Code.   See Toll Carrier List...
• Complete Street and Billing Address.
• The Type of Telephone Service Desired.  Residential/Business
• Other Credit Information, Including Employment, and Positive Identification.

 

A DEPOSIT MAY BE REQUIRED

 

A deposit will be required if you are not able to establish credit by meeting any of the basic credit requirements.  Residential Deposit - $50.00 -  Business Deposit $100.00.  A deposit is also required if your previous service with LCTS was disconnected for failure to pay the bill.   In this case you would be required to pay the outstanding bill, in addition, the reconnect deposit would be $100.00 Residential - $200.00 Business.

 

TO ESTABLISH CREDIT

 

A letter of credit from any other telephone company in the continental United States within the past six months; paid all bills on time during the last 6 months that you had service, and all final bills have been paid in full.

Residential Application

 

• $25.00 Installation Fee
• First Month Basic Charges in Advance $20.44
• Premises Visit – If Required - $15.00
• Deposit – If Required - $50.00/$100.00

 

 

Business Application

 

• $35.00 Installation Fee
• First Month Basic Charges in Advance – Single Line - $25.55
• First Month Basic Charges in Advance – Multi Lines - $27.58
• Premises Visit – If Required - $20.00
• Deposit – If Required - $100.00/$200.00
